 TIME:  1:00 PM – 4:00 PM

Town of Acton - 6 MONTHS AND ABOVE  (NO 65+ HIGH DOSE VACCINE AVAILABLE)
 
 LOCATION: Human Services and Senior Center
30 Sudbury Road, Rear
 
 DATE: Tuesday, October 11th
 TIME:  4:00 PM - 6:30 PM

** Please note ** Due to COVID-19 our clinics may look a little different, as we adhere to COVID-19 Safety Precautions as required by the Massachusetts Department of Public Health.

Masks are to be worn at all times per the Department of Public Health Guidelines for Healthcare Appointments

  • Please do not come to the clinic if you are ill or have any symptoms of COVID-19
  • Please make sure you come at your appropriate scheduled appointment time
  • Please practice social distancing while at the clinic
  • There aren’t any insurance forms to complete at the clinic, it’s all done online now via the link above
